  Last week on the Faceliftnet.com blog we took a look at dermal  filler Restylane, and how well it works to provide what is known in the  industry as a ‘liquid facelift’. This week, we’re looking at Juvederm, which is  a similar product (also a dermal filler administered by injection, containing  hyaluronic acid), but with a few fundamental differences.
Both Juvederm and Restylane are made up almost entirely of  hyaluronic acid; a compound found within skin. Collagen and hyaluronic acid are  substances within our skin that make it firm and youthful, so when our collagen  reserves recede as we age, our skin becomes saggy and wrinkled. Injecting Juvederm  into the subdermal layers of the skin will restore that youthful look, but only  temporarily, for up to 9 months. After that time Juvederm is absorbed into the  body harmlessly, so in that way it’s entirely natural and  safe to use. You can, however, opt for  Juvederm Ultra Plus, which essentially just lasts longer in clinical trials and  isn’t absorbed until 12 months later.

With the increasing popularity of cosmetic surgery, more and more  individuals who have previously undergone facelift and facial  rejuvenation procedures are seeking secondary revision facelift  procedures. Dr. Babak  Azizzadeh is a renowned revision facelift specialist based in Beverly Hills,  California.  He has been at the forefront of developing procedures to best address  patients who have previously undergone facelift and eyelid procedures.  Patients who present themselves for revision facelift procedures have  significantly different needs than the patients who have not had prior  facial rejuvenation procedures. As a result, secondary facelift  procedures require special techniques and care because of previous  surgical manipulations.
In the first portion  of the consultation, Dr. Azizzadeh will proceed with a detailed history  and facial analysis to best determine the appropriate surgical  procedures. Many patients have previously undergone not only a facelift,  but sometimes thread lifts, or other limited procedures that might have  distorted the tissues and will require special needs. During the facial  analysis procedure, five special areas need to be comprehensively  evaluated. First and foremost the entire overall skin condition and  facial balance will be looked at. The skin is an extremely important  part of the revision facelift consultation. Many patients who have  undergone previous lifting procedures have thinner skin and require  special treatment to avoid having complications from secondary  procedures. After the skin has been thoroughly evaluated, the facial  balance between the upper, middle, lower and neck regions of the face  must all be detailed. There needs to be a solid balance between the  forehead, brow, eyelid region and the rest of the face. Therefore, if  patients have any sort of imbalance still, the areas will be addressed  at the same time with an endoscopic brow lift and blepharoplasty  procedures. Also, we now know that the aging process is not just a  matter of gravitational laxity, but that significant volume atrophy is  also due to the change to the skeletal bone as well as subcutaneous fat  atrophy. As a result many patients undertaking secondary facial  procedures will require multilevel fat grafting to restore facial  volume.
Dr. Azizzadeh's main technique  with addressing revision facelift procedures in addition to multilevel fat  grafting, a blepharoplasty and endoscopic brow lift is  the utilization of the short flap rhytidectomy facelift, which provides  the best possible outcome for patients who have undergone previous  facelift procedures. The short flap rhytidectomy facelift (SFR  facelift) utilizes a short skin flap dissection, but this dissection  limits the risks of skin issues which is especially important for  secondary facelift procedure. The underlying tissue and muscles are  elevated with the skin flap in order to obtain the best possible  elevation of the appropriate areas that require improvement both in the  neck and the face. This SFR facelift furthermore allows fat grafting to  the midface and cheek areas, which are traditionally hollowed and not  addressing traditional facelift procedures. Furthermore, it also allows  chemical TCA peels to be performed with limited risks to the skin.
SFR  revision facelift procedure allows for a relatively short downtime with  patients returning back to work within seven to ten days. Bruising and  swelling is limited. It also limits the risk of injury to the facial  nerve especially for the patients who previously had had extensive  procedures.

Skin cancer treatment and reconstruction
Most types of skin cancer, when caught early, can be   effectively  treated using specialized methods of excision. Types of   cancer that can  be effectively treated this way include:

Basal cell carcinoma

Techniques used to excise the cancerous skin include:

Cutterage  and electrodessication-A technique in   which the cancer is removed using  a specialized tool called a curette,   which allows us to remove all the  affected cells with accuracy and   precision. Post-treatment reconstruction
Patients  who have had skin cancer are relieved when the   threat of cancer has  passed. However, many of them soon find that   relief tainted by the  embarrassment of the scarring that is left behind   once their treatment  is complete.
In most cases of skin cancer, the cancerous  material is   removed surgically. Depending on how far the cancer had  spread, and on   the area of the excision, this scarring may be minimal  and fairly   obscured, or it may be more troublesome.
Much of the scarring left after cancer treatment can be   treated with standard Scar Revision procedures.

Mary  Jo Buttafuocco had previously appeared on the Oprah Winfrey   Show  explaining that she continues to suffer from facial paralysis and   ear  damage from the shooting. After viewing the show Dr. Azizzadeh, a    reconstructive surgeon, said to himself "This is someone I could    definitely help. I decided that I was going to try to get a team    together to make her life better."
The bullet had traveled  through the temporal bone in Mary Jo's face.   The facial muscle that  connects the temporal bone allows for   expression and is connected to  the ear function. Dr. Azizzadeh says,   "The nerve was not completely cut  but was partially cut during this   gunshot wound; the nerve slowly had  some return of function but not   complete return."
The  reconstructive procedure was conducted in three parts by the   team. The  first of the three-part procedure was an asymmetric facelift   to restore  Mary Jo's smile and provide a symmetric appearance. Next,   Mary Jo's ear  canal was widened to improve her hearing and prevent   infections. To  complete the procedure Mary Jo worked with a physical   therapist to  strengthen her facial muscles.
Over the years Mary Jo had  not thought of having reconstructive   surgery because it was not the  most important thing to her. She says,   "My face was crooked and I was a  little self-conscious about it, but it   was something I lived with. I  was very blessed. I had my arms and legs   and brain and face." After Dr.  Azizzadeh approached her with the   changes that could be made to restore  her smile through reconstructive   surgery she was willing to give it a  try.
Dr. Azizzadeh's medical staff continually reassured  Mary Jo as she   was preparing for the surgeries. They asked her, "How do  you feel?" She   said, "I have been through a heck of a lot worse. This  is going to be   good. This is going to be fun. I don't mind getting a  facelift too!"
At the end of the procedures and at the conclusion of the Oprah   Winfrey show Mary Jo says, "I love the results!"
